A structured virtual full assessment focused on frailty care, covering: frailty and functional status, falls and mobility risk, cognition and memory concerns, continence issues, medication review, and advance care planning.
Short, focused medical consultations
These virtual consultations address specific health-related concerns. These appointments: are not for emergencies, do not replace a full geriatric assessment, and are suitable for follow-up or single-issue advice.
